In-Center Staff Registered Nurses

General Statement of Duties:

The staff registered nurse (RN) is responsible for the provision of quality dialysis care for the Puget Sound Kidney Centers (PSKC) patients and its documentation following established PSKC standards and practices and in compliance with the Washington State Nurse Practice Act.

Qualifications:

Education:

Evidence of graduation from an accredited school of nursing.  Bachelor’s degree in nursing preferred.
Licensure & Certification: Current licensure from the Washington State Board of Nursing. 
Experience: No dialysis experience is necessary due to our comprehensive dialysis-training program.

Skills & Abilities:

Essential Functions:

  • Punctual arrival to work on scheduled workdays.
  • Identify, plan, and implement plan to meet primary patient's dialysis needs.
  • Initiate hemodialysis treatment to assigned patients, according to PSKC procedures, using fistula needles, and central line catheters.
  • Maintain daily clinical records with accurate and legible documentation.
  • Provide primary patient teaching in the areas of:
    o Fluid and electrolyte balances.
    o Dietary restrictions.
    o Laboratory values.
    o Patient medications.
    o Hemodialysis procedures.
  • Monitor and discontinue dialysis treatment according to PSKC procedure.
  • Maintain accurate and current patient treatment care plan.
  • Provide appropriate intervention during an emergency situation.
  • Provide leadership and appropriate supervision to Dialysis Technicians.
  • Provide appropriate intervention during a hemodialysis crisis.
  • Maintain accurate and current patient medication lists on assigned patients.
  • Attend staff meetings and patient care conferences when scheduled.
  • Maintain communication lines between patient and medical team.  Refer patient needs to members of medical team as necessary.

Acute Mobile Hospital Nurses

General Statement of Duties:

The Acute Mobile Nurse of the Puget Sound Kidney Centers (PSKC) is responsible for dialysis care and documentation of dialysis care given to hospital patients.

Qualifications:

Education:

  • Evidence of graduation from an accredited school of nursing.

Certification & Licensure:

  • Current RN license from the Washington State Board of Nursing. Eligible for Nephrology Nurse Certification. Current CPR certification.

Experience:

  • Minimum of one year of experience in hemodialysis.
  • Minimum of one year of prior clinical nursing experience, preferably in critical care.
  • Experience in peritoneal dialysis and continuous renal replacement therapy (CRRT) preferred.

Skills & Abilities:

  • Ability to use every type of dialysis access with ease, understands their functions, and teaches others to analyze and use.
  • Ability to implement all modalities of dialysis safely and competently.

Essential Functions:

  • Respond to hospital’s dialysis needs per PSKC’s Mobile Contract.
  • Identify and implement plans to meet acute or chronic patient’s emotional and physical needs in the hospital.
  • Communicate patient status, both physiological and behavioral, to appropriate multi-disciplinary practitioners.
  • Initiate, monitor, and discontinue the following treatments according to PSKC’s procedures:
    o Hemodialysis
    o Peritoneal dialysis
    o Hemoperfusion
    o CRRT
  • Provide appropriate intervention during various medical crisis.
  • Assess patient’s physical and emotional needs.
  • Examine and document clinical findings.
  • Review laboratory data and dialysis orders.
  • Maintain treatment record sheets with accurate and legible documentation.
  • Ensure that all equipment is in working order and reports changes and repairs as needed.
  • Correctly interpret and execute medical staff orders for treatment in a timely manner.
  • Participate in meetings and task forces within the organization in order to contribute to process improvement, communications, and operations.
  • Participate in unit/organization performance improvement activities to ensure the development and implementation of processes that support continuous quality improvement, patient care, and effective work performance.
  • Participate in the maintenance of areas designated for dialysis in each hospital and its storage room.

Home Dialysis Nurses

General Statement of Duties:

The Home Dialysis Nurse (HDN) will provide assistance with home dialysis patients, training and managing both PD and HHD patients and their helpers at PSKC Smokey Point. This positions works under the direction of the Assistant Director of Nursing Services (ADNS) for Home Dialysis and Training.  The HDN is responsible for the training, medical management, the documentation of following established Puget Sound Kidney Centers (PSKC) standards and practices, compliance with the Washington State Nurse Practice Act, and the provision of quality dialysis care for PSKC patients who choose home dialysis as their modality of care.

Qualifications

Education:

Evidence of graduation from an accredited school of nursing.  Bachelor’s degree in nursing preferred.

Licensure & Certification:

Current licensure from the Washington State Board of Nursing.  Must be certified, or eligible to sit for, any of the examinations for the Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN), Certified Hemodialysis Nurse (CHN), or Certified Peritoneal Nurse (CPN).

Experience:

Experience either in hemodialyis and/or peritoneal dialysis is required.

  • Educate, train, and provide instruction for home dialysis patients and their Home Helpers.
  • Provide primary patient teaching in the areas of:

Licensed Practical Nurses

General Statement of Duties:

The Licensed Practical Nurse (LPN) assists and is responsible to the Puget Sound Kidney Centers (PSKC) Staff Registered Nurse for the care dialysis patients.

Qualifications:

The LPN is licensed by the State of Washington, and must meet the requirements of the Washington LPN Act.

Education:

The LPN is licensed by the State of Washington, and must meet the requirements of the Washington LPN Act.

Experience:

Acute care and or chronic care experience preferred.

Skills & Abilities:

 Essential Functions:

Punctual arrival to work on scheduled work day and advance notification to supervisor if unable to work regularly scheduled workday.

  • Prepare the dialysis machines and related equipment for the day’s use, utilizing PSKC policies and procedures.
  • Complete pre-dialysis data collection.
  • Initiate hemodialysis treatment according to procedure using dialysis access.
  • When performing the technique of venipuncture, or using a central line catheter, identify and communicate problems or complications to the dialysis trained Staff Registered Nurse.
  • Monitor and discontinue dialysis treatment according to procedure.
  • Identify, communicate, and intervene appropriately during Hemodialysis crisis and code situations.
  • Identify the importance of dry weight, and the significance of weight change, in the performance of a hemodialysis treatment, and report findings to the PSKC Staff Nurse.
  • Administer IV, IM, and PO medications according to procedure and physician’s order.
  • Understand the principles of dialysis and be able to use them when using equipment and supplies.
  • Use the principles of the aseptic technique appropriately in your daily job.
  • Identify and correct equipment and technical problems as trained.
  • Use appropriate written and verbal communication in the treatment area.
  • Assist in creating a therapeutic environment to maintain the psychological integrity of the patient and his/her family.
  • Participate in new employee preceptorship.
  • Participate fully in the continuous quality improvement of the PSKC delivery processes.
  • Handle appropriately the use of all hazardous chemicals.
  • Perform monthly inventory and maintain appropriate stock levels in assigned areas.

Nurse Manager - Clinical Education

General Statement of Duties:

The Nurse Manager, Clinical Education individual is a member of the Puget Sound Kidney Centers (PSKC) Department Head team, and is responsible for the design, implementation and maintenance of the PSKC Nurse and Patient Care Technician training and education programs. This individual educates and trains new patient care employees on the PSKC mission statement, employment policies, patient care policies and practices, patient education programs, and approved dialysis techniques. This individual also prepares and presents inservice educational opportunities to PSKC dialysis professionals.

Qualifications:

Education:

Bachelor’s degree in nursing (B.S.N.).

Certification and Licensure:

Current RN license from the Washington State Board of Nursing, and current CPR certification. In addition, a person certified as a Nephrology Nurse by the Nephrology Nurse Certification Commission must fill this position.

Experience

A minimum of five years experience in clinical nursing, and an additional three years of nursing care with patients diagnosed with End Stage Renal Disease (ESRD), including two years experience with hemodialysis and one year of peritoneal dialysis. Must have experience with, and an understanding of, ESRD rules and regulations. Must have knowledge and prior documented experience with the development and responsibility for an organized education program. Prior supervisory experience preferred.

Essential Functions:

Staff Training:

  • Responsible for the training of all new nurses and patient care technicians hired at PSKC.
  • Certify personnel who have completed the PSKC Patient Care Training and Orientation Course as competent to work independently as dialysis trained staff nurses or patient care technicians.
  • Responsible for development, implementation, and maintenance of a high quality patient education program.
  • Assess and evaluate all new clinical staff at 3,6,12, and 18 months.
  • Development and maintenance of an on-going inservice program for both nurses and patient care technicians.
  • Development of a program to prepare Registered Nurses for CNN exam.
  • Development of programs to prepare technicians and Licensed Practical Nurses (LPNs) for CHT exam.
  • Develop and maintain a method of providing contact hours for all staff on an on-going basis
  • Train all staff in CPR and all patient care and biomedical staff in the use of the Automated External Defibrillator.
  • Report to the Director of Nursing Services (DNS) of PSKC on the educational accomplishments and needs of PSKC patient care personnel.
  • This individual will be a standing member of the PSKC Policy & Procedure Committee, the Quality of Care Committee, and the Department Head team. Oversee Process Improvement Teams (PIT) decommissioned and active, providing annual summary report at Managers’ Retreat. Ad hoc member of Environment of Care (EOC) Committee

    • Back-up presenter for other mandatory inservices
    • Assure makeup offerings for mandatory inservices are provided
  • Coordination of staff testing for TB, blood draws for hepatitis B, and vaccinations.

    • Serve as HIV counselor for staff and patients
  • Ensure renewal of WSNA CEARP Provider status
  • Serve as a clinical resource by staying updated on literature/trends
  • Serve as a link between PSKC policies/procedures and clinical practice. Draft new policies and procedures.
  • Maintain working relationships with community and nephrology resources.

Administrative Duties

  • Assure that all administrative duties to support training programs are completed with assistance

    • Staff blood draw and TB results
  • Assembly of orientation/training manuals
  • Distribution of new/revised policies/procedures for procedures manuals and affected staff folders for review.

    • Assemble committee to select topics
    • Locate presenters
    • Assemble and distribute calendar of events
    • Distribute flyers announcing events
  • Coordination of approved outside educational opportunities for staff, including registration, counseling staff on travel, lodging, and reimbursable expenses, and submitting paperwork for staff reimbursement of expenses.
  • As necessary, communicate directly with the President & CEO regarding nursing and technician training requirements, and PSKC policy and procedure related issues.
  • Attend all mandatory organizational programs and maintain documentation for all unit specific certification requirements.
  • Participate in unit/organization performance improvement activities to ensure the development and implementation of processes, which supports continuous quality improvement and effective work performance.

Acute Mobile Registered Nurse - Part Time

General Statement of Duties:

The Acute Mobile Nurse of the Puget Sound Kidney Centers (PSKC) is responsible for dialysis care and documentation of dialysis care given to hospital patients. This position is part time up to 30 hours per week.

Qualifications:

Education:

Evidence of graduation from an accredited school of nursing.

Certification and Licensure:

Current RN license from the Washington State Board of Nursing. Eligible for Nephrology Nurse Certification. Current CPR certification.

Experience:

  • Minimum of one year of experience in hemodialysis.
  • Minimum of one year of prior clinical nursing experience, preferably in critical care.
  • Experience in peritoneal dialysis and continuous renal replacement therapy (CRRT) preferred.

Skills and Abilities:

  • Ability to work standby on-call for 24 hour periods. May be required to work up to 24 hours when needed.
  • Ability to use every type of dialysis access with ease, understands their functions, and teaches others to analyze and use.

  • Knowledge of and ability to apply universal safety precautions and infection control procedures. Skills and attributes sufficient to fulfill the job requirements and essential functions.
