Care Coordinator

Description: The Care Coordinator (CC) is responsible for the provision of quality care management for Puget Sound Kidney Centers (PSKC) patients, and for the documentation following established PSKC standards and practices in compliance with the Washington State Nurse Practice Act.

Qualifications: Degree from an accredited school of nursing.  Bachelor’s degree in nursing preferred.  Must have a current Registered Nurse license from the WA State Board of Nursing.  Current certification of Cardiopulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card.

Experience: Minimum of two years’ experience in Acute and/or Chronic Hemodialysis.  Prior experience with patient education preferred.  Prior experience with case management preferred.

Dialysis Technician

Description: The Dialysis Technician provides care for renal dialysis patients, following specific protocols, under the supervision of a Puget Sound Kidney Centers (PSKC) Registered Staff Nurse.

Hours: Ability to work any 4 days per week (Monday through Saturday) for 10 hour schedules. Able to work any shift between 3:00 AM and 12:00 AM, and rotate shifts. Able to maintain excellent attendance consistently.

Location: Ability to work at any PSKC facility: Smokey Point, Everett, Monroe and Mountlake Terrace, with possible occasional short assignments to Whidbey Island and Anacortes.

Qualifications: High school diploma or equivalent. CNA or MA is preferred. AIDS/HIV certificate desired. Current CPR certification required.

Experience: Medical or technical background relating to patient care or renal environment is desired.

Direct Care Manager  – Anacortes

Description: The DCM-Anacortes is responsible for the 24-hour per day supervision and management of in-center patient care. The primary responsibility of the position is to ensure that quality nursing care is delivered to each patient. The DCM is provided with the resources and authority to meet the unit’s objectives and is responsible for the proper management of these resources. The DCM is responsible for staffing assignments and takes call as assigned.

Hours: Typical Monday – Friday 8 hour shifts. Occasional weekends and hours outside of normal business hours.

Location: Anacortes.

Qualifications: Degree from an accredited school of nursing. Bachelor’s degree in nursing preferred. Must have a current Registered Nurse (RN) license from the WA State Board of Nursing. Current certification of Cardiopulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card. Eligible to sit for Nephrology Nurse Certification (CHN or CNN) or currently has this designation.

Experience: Minimum of two years’ experience in clinical nursing. Minimum of two years’ experience in Hemodialysis. Prior experience with supervision preferred.

Home Dialysis Registered Nurse

Description: The Home Dialysis Registered Nurse (HDRN) will oversee the training and care of Peritoneal Dialysis (PD) and Home Hemodialysis (HHD) patients at the Puget Sound Kidney Centers (PSKC). The HDRN will be assigned a group of home dialysis patients. The HDRN will be responsible for providing training, retraining, education, and nursing care to the home dialysis patients. The HDRN will provide appropriate supervision to Home Dialysis Technicians in the home department.

Qualifications: Degree from an accredited school of nursing. Bachelor’s degree in nursing preferred. Must have a current Registered Nurse (RN) license from the WA State Board of Nursing. Current certification of CardioPulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card.

Experience: Must have twelve (12) months experience as an RN. Experience in either hemodialysis or peritoneal dialysis required.

Home Program Manager

Description: The Home Program Manager is in charge of the home dialysis program at Puget Sound Kidney Centers (PSKC).  The Home Program Manager manages both new and on-going home patient training, education and treatments. The Home Program Manager is responsible for 24-hour per day supervision and management of Home Dialysis patient care. The primary responsibility of the position is to ensure that quality nursing care is delivered to each patient.  The Home Program manager is provided with the resources and authority to meet the PSKC Home Department’s objectives and is responsible for the proper management of these resources. The Home Program Manager is responsible for staffing assignments and patient scheduling.

Qualifications: Evidence of graduation from an accredited school of nursing. Bachelor’s degree in nursing preferred. Must have a current Registered Nurse (RN) license from the WA State Board of Nursing. Current certification of CardioPulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card.  National certification as a CHN, CNN or CPDN preferred.

Experience: Minimum of two years in Hemodialysis/Peritoneal dialysis.  Must have strong organizational skills, knowledge and experience in procedure writing and patient education and have proficient knowledge of the End Stage Renal Disease (ESRD) program, especially as it relates to home dialysis care.  Demonstrated experience in all modalities of dialysis care: Home Hemodialysis, C.A.P.D. and A.P.D.

Hospital Dialysis Registered Nurse

Description: The Hospital Dialysis Registered Nurse will be responsible for the delivery of dialysis to patients at local hospitals and other facilities contracted with Puget Sound Kidney Centers (PSKC).

Qualifications: Degree from an accredited school of nursing.  Bachelor’s degree in nursing preferred.  Must have current Registered Nurse (RN) license from the WA State Board of Nursing.  Current certification of Cardiopulmonary Resuscitation (CPR) with Basic Life Support (BLS) for Healthcare Providers card.

Experience: Minimum of two years experience in acute hemodialysis practice, including peritoneal dialysis, hemodialysis, extended nighttime dialysis and continuous renal replacement therapy (CCRT) preferred.  Preferred one year of prior supervisory or education experience.  Experience with Microsoft Office software suite, required.

Renal Dietitian

Description: The Renal Dietitian’s primary responsibility is to provide nutrition therapy to assigned Puget Sound Kidney Centers patients to promote optimal health.

Qualifications: Bachelor’s Degree in Nutrition Science, or equivalent. Must have registration by the Commission on Dietetic Registration by the first day of employment. Must obtain certification as a Board Certified Specialist in renal Nutrition (CSR) within 5 years from date of hire into the position.

Experience: Completion of at least one year clinical work experience in clinical nutrition as a registered dietitian.

Staff Registered Nurse

Description: The Staff Registered Nurse (RN) will oversee the delivery of dialysis and the care of patients at Puget Sound Kidney Centers (PSKC).  The RN will be responsible for providing education to patients, supervising Dialysis Technicians, adjusting treatment plans per standing orders, monitoring the patient’s outcomes, administer medications, participating in quality improvement projects and collaborating with other members of the patient’s care team (social worker, dietitians and nephrologist).

Hours: Ability to work any 3 days per week (Monday through Saturday) for 13 hour schedules. Able to work any shift between 4:00 AM and 11:00 PM, and rotate shifts. Able to maintain excellent attendance consistently.

Location: Ability to work at any PSKC facility: Smokey Point, Everett, Monroe and Mountlake Terrace, with possible occasional short assignments to Whidbey Island and Anacortes. Able to maintain excellent attendance and punctuality.

Qualifications: Degree from an accredited school of nursing. Bachelor’s degree in nursing preferred. Must have a current Registered Nurse (RN) license from the WA State Board of Nursing.  Current certification of Cardiopulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card.

Experience: Dialysis experience preferred but not required; comprehensive dialysis-training provided.  Prior experience in Acute or Ambulatory Nursing preferred.
